How to unlock the Big Brass Balls achievement in BioShock 2 Remastered - Definitive Guide
This seems daunting and unbearable but all you have to do is navigate through the gameplay options menu and disable vita chambers, and then play it as if it's an open world RPG (i.e. Fallout or Skyrim) and SAVE CONSTANTLY. Saves have now become your respawn point. Good luck, hope your game doesn't crash as much as mine does!

The first chance you have to disable the vita-chambers is after the first cutscene once you have started a new game. There is no option to disable the vita-chambers at the main menu, it must be done once you've started a new game. Hope this helps clarify as I wondered the same thing.

is it NECESSARY to actually disable vita-chambers from the menu to get this achievement, or can you get this by not dying in the game?

I would assume that you can still get the achievement if you don't die, even if the vita-chambers are still enabled. But that would just be an unconfirmed guess on my end. I would still recommend disabling the chambers as soon as possible, as dying once, even accidentally, would void the achievement.
