This is achievement is very straight forward. Complete "Worlds of Hurt" in any time period on any difficulty level. Any combination of weapons and plasmids may be used.
To complete "World of Hurt" you must complete all 8 arenas and the final assault in the main lobby to free the Little Sister.

Worlds of Hurt does not involve any puzzles or anything like that. It's basically 8 challenge rooms where you have to kill all enemies to continue, without dying as there are no saves, and no vita-chambers around.
When you've killed all enemies around, using whatever assortment of weapons, plasmids and tonics you may choose you can save the sister and be victorious.
