Complete the game on the hardest difficulty setting.
*Spoilers*
This achievement is really easy. You can play on the difficulty Easy until the final area where you fight Atlas. This method can be found on easily on the internet. I see there are 2 Guides for it here, but they aren't very detailed so I will write one in my own words. You can also get "Brass Balls" but you have to have the DLC. Make sure you read the IMPORTANT section (If going for "Brass Balls") and do the following:
1. Start a game on Easy, and play through (you can go for any achievements you want while you do) until you get to the very end of Proving Grounds (The second last area). You will be a Big Daddy and you will be with a Little Sister. When she climbs in the hole and give you her weapon to steal ADAM, you are at the end of the game.
Note: Do not switch the difficulty at any other time in the story, because it may affect you not getting the achievement. Read the comments below for more information.
2. This is only if you are going for "Brass Balls". Press Start and go to Options. At the very bottom disable Vita-Chambers (it would be wise to disable them in the very beginning of the game; read the IMPORTANT section for more information).
3. Go into the Elevator and you will Ascend. You will then be in a new area "Fontaine". This is where the Boss is. Fight him normally, draining his ADAM. You have to drain his ADAM four times.
3. When you go to click "A" to drain his last bar of ADAM (the 4th time) after you click "A", wait half a second and then click Start. Go to Options and turn the difficulty to Hard.
4. Now this is VERY important. When you turn it to hard, click START! Do not click B to return. Make sure you click Start.
5. After a few seconds you will go into a cut scene. At one point in the Cut-scene you should get the achievement. If you are going for Brass Balls that achievement should unlock too.
6. Wait for the whole cut scene to finish. If you don't have the achievement, load your game up from the last auto save and repeat steps 1-5. You will be coming out of the Elevator.
IMPORTANT: Make sure you do the following because if you don't you MAY not get the achievement "Brass Balls".
1. Don't Die in the game. This should be easy on Easy. Unless you disabled Vita-Chambers from the very beginning, you should be able to die and still get the achievement. But to be safe, try not to die.
2. Make sure you have the DLC and you are connected to xbox live (you need the update).
3. Make sure you Disable Vita-Chambers before going up the elevator. (Even the smartest people make mistakes)
4. Make sure you change the difficulty to "Hard" when you are Draining his last ADAM bar.
5. Make sure you press the START button after changing the difficulty. Do NOT press "B".

The reason this achievement is easier than it sounds is because of the vita-chambers. As long as you have vita-chambers turned on you will come back to life close to where you died. The only place you will not respawn is while fighting the final boss, which will prove to be a challenging fight.
I suggest starting the game on hard and playing through. If you feel you're up to it I would also suggest going for as many achievements as possible, on my playthrough I got 49 of 51 achievements, not including historian or Brass Balls.
So start it up on hard, and don't be afraid to die. Infinite lives makes a seemingly hard achievement all that much easier.
Although like I said, the final boss is quite a challenge as you will not revive, so here are some tips:
1) Make sure to have as much ammo and eve/health items as you can carry.
2) Don't hesitate to heal. If you have to die, at least go out fighting with no more medkits knowing you gave it your all.
3) Use the crossbow. It does massive damage and is very accurate (reload time is a bit long, but the damage is worth it.)
4) Learn the boss's patterns. Once you do you will be able to defend against them.

5) Do NOT wait to drain his adam, he will come back down and start attacking again meaning you have to run through the sequence again. (I made this mistake and learned this first hand)

6) Use the Electric gel on the first 2 sequences as it will make short work of the boss in these sections.

1)Like Quarantine said, learn to use the crossbow throughout the game, with the weapons upgrade the bolts can be retrieved from the bodies. Head shots to most splicers are one shot kills.
2)With the dlc there is an Eve tonic that lowers the Eve cost to plasmids. Also, search everywhere and sometimes just blindly shoot Telekenesis around, you might get lucky.

3)Photograph the Spider Splicers as soon as possible, their fnal level bonus makes looting them worthwhile.
Endgame FONTAINE
During Proving Ground collect all the ammo and money you can, remember, the Little Sisters are expendable, the game doesn't end when you lose a set amount (I lost a few). After Proving Ground you will have an opportunity to buy or make what you need and you can always leave and come back until you get on the elevator. Once on his platform there is a Gene machine far around to the right, you'll need this.
Now for Fontaine, before you stick him with the Adam needle use the Gene kiosk, I found Electro Ball, Incinerate, Winter Blast, and Sonic Boom (from the dlc), helped.
Fontaine changes form after every syphon, 1st he's fire (use ice), then he's ice (guess where this is going?). Here's were Sonic Boom helped, After the first go around he'll bring in a bot, then some splicers and the Boom clears them away for a little.
Ok, conventional weapons: I set up a line of proxy mines, then used granades to take one "life". Next, I unloaded with the flame thrower. Also, what I found extremely usefull was antipersonnel machine gun rounds and armor piercing machine gun rounds (try to stay away from the pistol, loading time vs overall damage is bad).

The glitch is
Play the game on any difficulty and just before you have to drain the ADAM for the last time press start and change difficulty to hard then press start to return to the game (do not press B)
then drain the ADAM
this will unlock the cheevos for hard mode and brass balls(if you did not use a vita chamber that is)
they will unlock after you awake during the last cutscene so dont freak out if they dont pop instantly
I went back and tried the change difficulty trick and it didn't work.
So i deleted everything including the game and reinstalled then downloaded the dlc and update and started on easy.
Completed the game then tried the trick again and nothing.
I then deleted everything again and played through on hard
Started on hard and had the vitachambers turned off plus I saved all the little and never entered the options menu again and nothing.
I have beaten fontain multiple times and nothing so for some people these two achievements are impossible to get.
You should start the game on Hard, seeing as you have unlimited lives. When you die, you are reborn at a Vita-Chamber and the game stays exactly how you left it, meaning every enemy will have the same amount of health if you were in a battle. You can essentially run at a Big Daddy and melee it until it kills you, then get reborn and return and do the same. It is impossible to get a "Game Over" so to speak, so this difficulty is really not hard at all.
This achievement is earned for completing the game on the Survivor difficulty setting. For BioShock veterans of the Xbox 360, this will be new to you since this difficulty was originally unique to the PlayStation 3 version of the game. This remastered version of the game now includes a New Game+ feature that is unlocked after completing the game on any difficulty. For this playthrough, make sure that you select New Game+ as well as Survivor difficulty. All of your data like Weapon Upgrades, Inventory, Research, etc. should now carry over, making the beginning of the game a lot less frustrating. All of these items should unlock once you reach the Medical Pavilion.
There are two different ways to complete this achievement: beating the game from start to finish on Survivor difficulty or by using the exploit described in Step Four of the Abbreviated Walkthrough. For the purposes of this guide, I will be giving tips for players that wish to playthrough the entire game on Survivor difficulty.
Survivor difficulty is not much different from Hard, but can be a little bit of a slog. If you are following the Abbreviated Walkthrough of this guide, this will be Step Two. Since you should have unlocked all other campaign achievements during Step One, you should now have a lot more freedom to play the way you want during this playthrough. You do not need to force yourself to take on every Big Daddy in the game in order to earn ADAM from the Little Sisters. You should only rescue as many Little Sisters as you think you need in order to upgrade and purchase Plasmids and Tonics. If you only use a couple of Plasmids, then don’t worry about purchasing additional Plasmids or Plasmid slots since that would be a waste of time and ADAM. There is no point in using valuable resources fighting Big Daddies if you don’t need the ADAM.
It is still highly recommended to complete all of the research using the Research Camera (if you hadn't already during your first playthrough). Through research, every enemy will eventually grant you a couple of damage increase bonuses. These are critical to playing the game on Survivor difficulty and will make the later levels of the game more tolerable.
Due to ammo being in short supply, it's also recommended that you visit all of the Power to the People weapon upgrade stations (if you didn't during your first playthrough). It may seem like a little bit of overkill, but you never know when ammo will be in short supply and you may need to use a weapon that you normally don’t. I personally used the Machine Gun, Crossbow, Chemical Thrower, and RPG (all fully upgraded). A fully upgraded Chemical Thrower with full ammo can make the final boss fight a cakewalk. The Electric Gel freezes him in place and does a pretty fair amount of damage.
I think the most important thing that I recommend for your Survivor playthrough is to research the Spider Splicer as much as possible and as soon as possible. The second level of research for the Spider Splicer grants the Natural Camouflage Tonic. This Tonic grants you invisibility when standing still. Enemies will completely ignore you when you are invisible.
Scavenge. Scavenge. Scavenge. The more cash and supplies you have, the easier this playthrough becomes. It's always advised to have as many Health Kits and EVE Hypos in reserve as possible.
Use the Vita-Chambers. Since you already beat the game on Hard without using Vita-Chambers in Step One of this walkthrough, you are free to use this respawn system. Every time you die, you will respawn in the nearest chamber and any damage that you inflicted on enemies will carry over. However, since respawning will cost you some dollars and some EVE, it may still be better to save often and simply reload a save.
