Basically unmissable. Just keep talking to your companions when you see the ! symbol at your home. Remember to select every option. You should also explore every possible node when setting off. I unlocked this in chapter one.

Just an addition to the above, be sure to explore all dialogue choices when exploring areas, some choices have encyclopedia rewards in them. Make sure to save beforehand and explore all options. Also, some areas you are in direct control of your character and can run around like in the tutorial. Examine all the white dots as well, some have encyclopedia rewards in them.
