You will be in the cellar with the Dr.
interact with the maid and the cellar will become red and you will have another vision. You need to interact with the vision at 3 certain points. Like all visions you can die, when the screen darkens just move away or you will die
Point 1 - When the two first start talking
Point 2 - He rips her dress
Point 3 - he threw her pendant in the tub
After all three are examined you will have another seizure afterwards the achievement will pop

Story related, impossible to miss.
Chapter Three. Once you have examined the crime scene, the environment will turn red and ghosts will appear just as they did in the Chapel. You must examine the scene that plays before you at three set points in order to further the story: as they hug, as they start to fight, and as the woman is pushed away.
You can die during these visions, so be aware of when the screen starts to become black. Moving away from the vision will halt the danger.

Story related. I suppose you could miss it, if you keep dying and then just quit the game... but all you have to do is interact with the vision in a timely fashion and move slightly away from danger before interacting again.
