After making your way to the village you use a plank to enter a boarded up cavern. After interacting with all the documents you will have to reassemble a tornup page in the cavern straight after this you will have your first vision, just examine the vision infront of you
the vision will reced and you will have to trigger vision 2, head through another tunnel into an area where you will see vision 2 of a ghost playing with the lamp, examine this.
now head up the stone steps and interact with the altar which was in the prologue to trigger vision 3.
After this the achievement will unlock.

Chapter Four. After doing some investigating inside of the abandoned village, a vision will start. Unlike the previous ones, this is split up into three areas in the village which you have to find. The first is in the area the vision automatically starts in, the second is outside and to the right as you follow the path (under the two arches), and the third is inside the stone circle at the top of the hill.
You can die during these visions, so be aware of when the screen starts to become black. Moving away from the vision will halt the danger.
