This requires you to get the morality bar either fully to the left (Rotten) or right (Noble). Once you have got your morality to one of the extremes you then need to complete three additional tasks. These are random, so to see what they are select the moon (evil) or sun (good) at the end of the morality bar and press 
to show info. There are three moons or suns above the main moon or sun which light up when each of the three additional tasks are complete. The achievement will unlock when all three are lit up.
Your choice of good or evil at the start of the game doesn’t matter. I chose good at the start but ended up being as evil as possible.
