SPOILER ALERT
This achievement can be done easiest on the last level the Dragon Temple. Once you've turned into your dragon form you will come into a room with a massive enemie spawn. Basically you will want to gather 10 or more around you which can be done by just waiting in the middle of the room. You will need to kill/attack some of them to fill your rage meter to second stage and then activate your lvl two heavy attack by holding B and pressing A afterwards.
