After opening the gate to the top of the saw mill, you will find a lumberjack dead with a sawblade in his noggin. Take your phone out and go to contacts and call the VA. Simple as that.

This is missable.
Once you've been able to reach the sawmill in chapter 11, you can get through the gate with code 3324. (This is written down in a locked room that you can access through a red tape in the opposite building.) Then...
As soon as you see that, pull up the number for the VA in your phone book and call right away. If you leave the mill, your signal will drop and you won't be able to make the call.

This is a missable achievement. Watch the tape called The Chase simply have the guy running away open up the 2 doors for you. In the second room you can find the code for the game that is written on a note. Simply make your way back to the gate, the code is 3324 go up the stairs and go to the injured solider, than a cutscene will happen simply HOLD RB and select your phone and call the VA Office.
