All good my dudes smile and yeah, on my first playthrough I found every psychologist file bar for #4. I managed to find it on my second run through but after looking on the forums after posting the guide I noticed a lot of people mentioned not being able to find #4. I think it eluded near enough all of us for a while laugh

Buggy? This achievement along with Garbage Man haven’t unlocked after collecting all 9 and 15 of each collectible respectively

If the game shows you have all of the notes in your backpack but you don't unlock the achievement, simply exit and close down the game and reload checkpoint. I did this and it popped right away upon collecting the last note again.

If you had all notes and didn't immediately reload your save at the closest checkpoint to the last note (like me). Then just start a new game and pick up the picture by the police car in the prologue. Course you will probably also need to do what Tukraze said and close and restart game.
