There are 3 weapon upgrades in the tunnel stage. You simply need to fly in to them, they all appear near the top middle of the stage as you fly.
Once you get the 3rd one, this achievement will pop.

I think this achievement is actually cumulative, at least within a single playthrough. This popped for me after grabbing the first two upgrades, as I'd grabbed the third one (after missing the first two) on an earlier continue.

This achievement is trivial but I was unclear based on your explanation.
There are three levels of weapon power on this level. You gain a level when you pick up a weapon powerup and you lose a level if you die. This achievement simply requires getting to level 3, which would happen with the second powerup if you didn't die and it took you to weapon level 3.

This is part of mission IV and is the last area of the mission. You will be flying through a 3D tunnel for this section and must dodge enemy fire along the way. Spread out through the tunnel are three blue crest pickups, collect them until you get the third weapon icon on the left of the screen. This only requires that you pick up two if you are playing on easy difficulty as you already start with one.
