For this achievement, you need rare (blue) or better gear in every slot, including the necklace and ring. The necklace will likely take quite some time, I didn't get one until I got a Crimson Necklace from Altar of Blood, which is a level 29 dungeon. By the time you get a necklace you should have rare everything else.
I do want to note that the in-game trial (which is also part of the second campaign) does not seem to unlock when the achievement does. It appears you need either all blue, or all blue and +0 enchant level for the trial to unlock. I did not get the trial with the achievement and had to use a bunch of blue gear to get it to unlock.

True I had the same problem I was wondering where I went wrong then realised it had to be specific all blue it was worth getting it done for the bag and storage expansion ticket to complete that set of 13 objectives since I was often finding myself running out of inventory space mid game onwards

Really easy now if you are going for level 40 like I am. In the mail you get a bunch of high level gear which "should" meet these reqs. If someone is 40 and doesn't have this please check for me. Im half way there Lol
