It seemed like everytime a play was ran that a person would get injured when playing the prison game after beating division 3. This is how I got the achieve.

The, Pants, Party
First thing you want to do is get some icons to fill all your slots and get your Unleash bar. The quickest way to do this is just punt the ball away and use dirty hits and beatdowns while on defense. Now when you're on offense, take Franchise and enter Unleash mode, then hit up on the right stick when you get close to a defender. He should leap over them, knock them over, or something to that effect. It doesn't always injure them, but there's a good chance for them to get a concussion or injure their foot, sometimes even some worse injuries. You'll get this when going for the secret achievements.
