Easy method
Create a Goblin Team - 16 Goblins, Apothecary
Create a Dwarf Team - 9 Blockers, 2 Slayers
Make sure that the Goblins are at least 200TR below the Dwarfs. Take less Goblins if necessary.
Start a One-off game between these teams, with you as the Goblins. Choose two Wandering Apothecaries for Inducements.
When the match starts, the only action you'll take will be to stand your players up and move them back into tackle zones (if needed). When you inevitably start taking casualties, choose to use the Apothecaries. Once you've used all three, the achivement should unlock.

i cant get this achievement to pop. i have used 3 apothecary's in a match numerous times and no achievement.

I know this is ancient now, but I'd definitely edit the solution.
"Try using the Apothecary on Badly Hurt players, then keeping the original result."
This is the only way I got it to pop personally. Any actual injuries (broken jaw, smashed knee, back, etc.) never produced a result that allowed the player to return to the game. On a "badly hurt" only player, I used the Apothecary, but always selected "no" so that I used the original result. The player (I believe) just makes his way to the sideline at that point and re-enters after the next break in action.
Also important to note: This must be done in Classic, not Blitz. The Wandering Apothecary inducement is only available in Classic Mode. (For noobs like me who primarily played the real-time, blitz setup)

To get this you will need to have an Apothocary on your team and then spend 200,000 GP's on extra Apothocarys from inducements.
Then just leave some rookie team members in places you normaly wouldent, like infront of a Troll for example.
When they get injured just agree to use the Apothocary every time and 'plink' Achievement unlocked.
Note - If you save before entering the match you can just play until you get the Achievement and then load back to before the match and still have the 200,000 GP's
