To begin, to my knowledge all achievements need to be done in a campaign game, that said continue with the following:
Start a new campaign on Easy and Blitz (the live action us easier to complete this on) as either Wood Elves or Skaven.
When playing your first opponent hopefully you get the Goblins or another Wood Elf or Skaven team, choose to play one if the above listed 3 (avoid playing Chaos or Dwarfs!!!!! They'll beat your ass, literally!!)
Once in the game as soon as you are able to recover the ball, either through a turnover, receive a kickoff, whatever get the ball to your thrower asap. If playing Elves have a catcher close by if Skaven a gutterrunner. Then simply throw the ball.
Some notes, it's easier to constantly use concentration to get players positioned and get the ball back. Also score quickly to get the ball reset again.
Alternately you can keep the ai off your thrower and simply pass back and forth repeatedly. Remember that in blitz it will take a moment to reset your time between throws so be mindful of this if you choose to "play catch"

Go for this and 'Team Play' together.
Simple enough, if your having dificulty use a Wood Elf team, Hand the ball off to the teams Thrower, he will then pass it to the same guy.
Next turn rince and repeat until it pops.
Wood Elves have AG 4 which means they make Quick Passes on a 2+, this coupled with the Throwers 'Throw' skill means he can re-roll if the pass is failed the first time.

Find a player on your team with the "Pass Skill" and use them for this achievement. Try to pass to someone with the "Catch Skill". Keep passing back and forth and you will soon unlock the achievement.
Note: If you drop the pass, or fail the pass roll, that will be the end of your turn.

Choose campaign, easy, real-time, Skaven, 2 Throwers, enough blockers.
At the start of the game choose to receive the kick or wait until you are scored against.
Position both your throwers (Thrower 1, Thrower 2) near the back of your side of the field.
When the ball lands from a kick to you, go into concentration mode instantly.
Set Thrower 1 to pick up the ball, exit concentration mode.
Once Thrower 1 has the ball enter concentration mode.
Select Thrower 1 to throw to Thrower 2.
Check your blockers to ensure the other team isn't coming near.
Exit concentration mode and hope for a successful pass.
Throw back and forth between Thrower 1 and Thrower 2 by entering and exiting concentration mode to ensure minimal movement of the other team towards your throwers.
Eventually you will get it.
