How to unlock the Nightmare Lecture Building achievement in Bloodborne - Definitive Guide

After defeating Vicar Amelia, head out of her area and descend the stairs, taking your immediate left path as you reach the bottom (taking out the two Frenzy inducing guys on the stairs). Follow this all the way along, taking out 2 Hunters (who do not respawn) and several other enemies along the way. At the end of this path is a large church/cathedral looking building, and as you approach it you'll probably think "Oh yeah, there's a boss in here for sure". Well, you're right, but not like you think. As you walk into the room, nothing will happen. I even stood on the altar in the middle of the room thinking it would trigger the fight, and nothing. Go to the door on the other end of the circular room, however, and you'll be instantly Frenzied and lifted into the air, much like you may have had happen earlier when you picked up some armor. Only this time we see what was doing it to you. After a short cutscene you're transported to the Nightmare Lecture Building and the trophy will pop.

Edit: Thanks to the commenters for confirming that you need the Tonsil Stone for this to work. The Tonsil Stone can be found about halfway through the Forbidden Woods near the dog cages. Right before that spot you'll see a house with an incense candle, speak to the person in the house and they will give you the Tonsil Stone.

I think you need an item before it works...but I am not sure if this item I am thinking of is used elsewhere

If that is the case I'm unaware of which item is needed, but would happily add to the solution if anyone can confirm. The steps listed above were exactly how I got it.

You need a Tonsil Stone, which comes from talking to someone in one of the houses with a red lantern outside, after the fight with Vicar Amelia.

I got mine from the house with the dog cages in the Forbidden Woods.

An alternative (read: overly complicated and stupid) route to getting this Trophy to pop is to beat The One Reborn, at the end of Yahar'gul, Unseen Village:

Once he's down, the room beyond his contains a dead NPC on a chair who will teleport you to the Lecture Building. No idea if you need the Tonsil Stone for this too, though.

The location itself is located to the right of the Main Cathedral, there first we will meet two “defiled” hunters, having dealt with them we need to follow further to the Old Church which is located below the path, in the main hall of the church there is only one door when we try to open which we are killed by a huge creature barely visible in silhouette and it is in we need to get into this door... So, we need an Almond stone to get through this door, this stone will be given to us in the same window where we talked with the girl to get the box... what conditions must be met so that the girl is replaced by a woman who gives us a stone, I didn’t understand...

Zabzan is wrong - there is no need to kill the Roma, the main thing, as written above, is to get the almond stone, and then, when you approach the door, a hand will grab you and take you there.

The almond stone can be obtained in the passage that is located immediately after passing the gate for 10k.

We turn left, kill two hunters and go downstairs, turn left to the passage to the church and along the way we talk to the people behind the doors, one of them will give this stone.

The stone is given behind any door in central Yharnam after receiving 40 insight points. I got it from Gilbert

After Amelia has been killed and the lamp in the Forbidden Forest has been activated, you need to talk to any non-key NPC through the window (a small reddish lantern is lit next to such windows). If the red moon has risen and when you try to talk to an NPC, the message "No answer" is displayed, then the stone can still be obtained. To do this, you need to go to Gascoigne’s daughters and Patchwork Spider will talk instead of them, who will give the stone.

An almond stone is given by an old man in the forbidden forest (look at the red lantern) after a conversation, past which we pass by a cave with a poisonous lake along the Cainhurst trophy (above video)

No one is wrong, Almond Stone can be obtained waaaaaaaaaaaaaaaaaaaaaaaaaaaaally in any window (with a red lamp). The easiest place is central Yharnam, right next to the lantern (there will be two doors right in front of it, and a window on the left, that’s exactly where you need to go). By the way, I advise you to visit this window immediately after killing either the Bloodthroat Monster (in Old Yharnam), or after killing Father Gascoigne.

I received the Almond Stone in the following place: we run to the tower of the Healing Church Workshop location (the right door from the lantern in the Cathedral District), but after the bridge with enemies shooting from above, we do not enter the tower, but go around it in a circle. We half-jump through the gap to the tier below, go through the door and carefully, jumping from ledge to ledge, we descend to the very bottom of the tower. We kill the werewolf, open the door, and immediately on the right side of the street there will be a red lantern. We speak with the inhabitant and receive an Almond Stone. At the end of the location there will be an elevator to the top.

the so-called Tonsil Stone can be obtained from ANY NPC with a “red lamp” (you will be greeted by another voice and will offer you the stone) after defeating Vicar Amelia AND at least one visit to Forbidden Woods. But after defeating Rom, The Vacous Spider, the stone can still be obtained from a person in Forbidden Woods, not far from the cages with dogs.

The easiest location to get the stone is Gilbert - the NPC that is located right next to the Central Yharnam lamp

important: the NPCs whom you sent to “safe places” will no longer be able to give the stone.
