I can't remember the exact pre-requisites. I will have to playthrough again and see what the triggers are.

I got it to pop by sneaking around instead of doing a stealth takedown

So I tested with an alt and knifing him also unlocks the achievement. The important thing is to choose FAST when sneaking past the bathroom in order to get him to come out.
