This is now obtainable as of 11/19/21.
First, when you find the mines at the bridge you need to save Gav.

After some dialogue with him (I don't think these choices matter) you will be walking through the woods and get ambushed by Reah.

>Spare Reah
She will now reappear and kill you. After the rewind choose

Now it will fast forward and she will appear again. This time Gav will shoot her and the achievement unlocks.

I couldn't get the scene where Reah reappears and kills you during a romanced Gav playthrough, Gav would immediately shoot her. Did another run, saved him on the bridge and ignored him otherwise, and the correct scene played and cheevo popped
