For this achievement you must finish all story levels and then all subsequent game modes with an A ranking in each level.
Fortunately, these can be done in any difficulty, so obviously it is best to tackle this on Easy.
Just to reiterate, this achievement has nothing to do any progress related to loans, credits, purchases, beating the game on all difficulties or the misc. achievements. Just the game modes.
Along the way, you will unlock "Living on a Prayer" for hitting the 50% mark and "hung in there" for hitting the 75% mark for the same requirements.
