Beat the game on hard. You only need to do story mode and you do NOT need to get an A rank.
After getting A's on all modes of easy this should be easy. Like half hour easy. The last level of Pizza is a little challenging. The biggest differences in Normal and Hard is that you blow up easier and the goal areas are much smaller.

Not much of a solution, but I figure I would add this due to the semi-cryptic nature of the achievement title/description.
This achievement is unlocked by beating the game in Hard mode.
Hard more is available from the very start. Each difficulty has its own progress tracking, so you can switch difficulties at any time and it won't affect the progress of the other difficulties.
The mode itself is not really that difficult, although you may want to play through on Easy and Normal just to get accustomed to the controls.
You only need to play through the story mode levels and there is no requirement for the grade you receive.
