The Rare Snow can be bought from Mork in Stoneheart City for 1200 Orbs (The entrance is marked by a giant rocky book).
Give it to Nuno, who's in a bed complaining about his stomach hurting, inside a house in Rust Town.

At some point buy the Rare Snow from the bookkeeper in Stoneheart City (there's a book carving outside his door).
Later when you are in Rust Village look for the sickly-looking boy in one of the houses, simply give him the snow and the achievement is yours. Luckily you don't have to worry about the snow melting, I had it in my inventory for most of the game.
