Telefrag is when you kill someone with the actual teleport not with the boomerang. At first I though u had to teleport right after killing someone.
Easiest way to do this is to make a game with 2 players with only the teleport power up active. Throw your boomerang and let it lay somewhere on the ground, let player 2 stand on top of it and teleport.

When you have the teleporting power-up use it to teleport to your boomerang while someone is standing or walking over it. Should be not too difficult to get with many bots on easy.
