How to unlock the Chocolate Chip Confirmed achievement in Borderlands 2 - Definitive Guide
*WARNING THIS IS MISSABLE, IF YOU DO NOT LET TINA LAUNCH THE ROCKET YOU WILL HAVE TO RE DO THE WHOLE DLC LIKE I DID*
During the mission 'Shooting the Moon' Tina will task you in collecting some parts for the rocket. You'll collect two electrical parts as well as two wings in which you'll place onto the rocket. She will then task you to go collect a power core from defeating 'Uranus Bot' and them you'll have to place that onto the rocket. Once you have done this you MUST talk to Tina to let her launch the rocket as the other option is to launch it yourself making this achievement void. After she launches the rocket, you should unlock the achievement! If playing in a multiplayer game or split screen, all players should unlock the achievement as well!
Video Guide Reference if needed:
Happy looting!!
*EDIT* Via Sons O Liberty - For anyone not getting the achievement or any of them in the new DLC. Go to settings on your XBOX ONE, remove account, add it again, let it download your information again, , load Borderlands 2, instant achievement pop, I've had to do this for almost every achievement.

During the mission "Shooting The Moon" after collecting all the parts for the missile you have the option to arm the cannon or you can let Tiny Tina do it instead. For this, you will want to talk to Tina when you are given the option. Upon doing this you will get the achievement.

Looks like I will have to do 2nd playthrough thanks to lazy scumbags for given us the option on screen.

Watch my buddy mash the button to launch the rocket right in front of me...worst part is I told him to look out for it in the area, as I figured it was a missable one...guess we're running through again..

Well, other gamers have a better solution, I´m just here to tell you that if you are playing co-op (i played split-screen) both of the Gamertags will unlock all the achievements from this DLC.
I was helping a buddy, and accidentally he let Tiny Tina shoot the rocket and we went to our profile, select borderlands 2 and the achievement pop, both were happy.
Happy Hunting Fellows.

*MISSABLE*
On the quest "Shooting For The Moon" you'll have to go around collecting parts to attach to a rocket. Once you acquire all of the parts you must talk to Tiny Tina and let her launch the rocket instead of doing it yourself. If you happen to miss this you will have to replay the DLC like I do since there is no way to replay missions.

This achievement is missable.
You will unlock this achievement during the side mission "Shooting the Moon."
Towards the end of the mission, you are tasked with helping Tiny Tina build an explosive rocket. After you have collected the parts for the explosive rocket and assembled them, you will get a secondary objective, which will allow Tiny Tina to arm the explosive instead of doing it yourself. When you get this secondary objective, instead of activating the explosive, speak to Tiny Tina and she will arm the explosive for you, thus unlocking this achievement.

Easy to miss!!! When you are on Hyperion for the first time, there will be a mission to collect spare parts for a bomb. When you assemble it, you will need to press a button. DO NOT PRESS. Let Tina press. Talk to her and she will press it herself. Afterwards they will give you a trophy.
