How to unlock the Gone From My Sight achievement in Borderlands 3 - Definitive Guide
This achievement is story based and should be completed during natural progression. Beneath the Meridian is a main mission that chronologically occurs as below:

Sanctuary
Hostile Takeover
The Impending Storm

Beneath the Meridian
Essentially, start by traveling back up to Sanctuary and handing the final fragment of the Vault Key to Tannis in her lab. Report now to the bridge to finalize the plan on looting the vault and fast travel back to Meridian Metroplex. Grab a vehicle and make your way NE with some reinforcements which include Zero. Take out the baddies along the way so Zero can lower the shield letting you enter Neon Arterial.
Before heading on, grab the Typhon Log on the left overlooking the water. You'll meet up with Maya next to the Fast Travel and jump aboard Project DD courtesy of Ellie. Follow the relatively straight forward roads to Apollyon Station fighting/fleeing bandit vehicles along the way. Maya will also attack any that get close to you. There will be two instances in which you must clear out the area of baddies so Maya can open the roadblocks. Eventually, you'll end up at the train station where you can load up.
Work your way into the station through a big battle (there are some good chests in the container tower on the left side) and grab the 2nd Typhon Log on the near right side from where you entered. Once Maya blasts open the door, you''ll fight through several rooms with waves of enemies pouring in at you. You'll get to the tracks in which a train will unload more bandits at you and then the train will occasionally pass on through so be sure not to get hit by it. You'll also see a Claptrap on the right side of the tracks.
After passing out of the station, you'll descend into a tunnel where you'll meet your first guardian. You'll be battling these guys the rest of the way just bear in mind they can move quickly so keep some distance. After you pass a waterfall coming from the ceiling there is a slight branch on the left with the last Typhon Log. Since this is the 3rd one, the drop will appear on the other small branch from the main cave just a little but further up on the right. After this, you'll have one more big fight before entering The Forgotten Basilica.
Make sure you gear up now and clear your backpack because it's time to fight the Vault Monster - The Rampager. Slide down the ramp into the basilica and activate the pedestal near Maya then brace yourself. Some tips for this fight is to simply keep moving to avoid his projectiles, fire at him when he's up high and unleash everything when he is stunned. There will be occasional guardians popping up to help as well as these flying spheres that protect him so make sure to address these as well. When he's finally down, grab the loot and head into the vault. Head straight to the icon and grab the Eridian Resonator which will allow you to finally smash those eridium crystals you've seen around. Do so on the chests here and grab everything else before heading out.
You'll meet up with Ava and Maya and trigger a cut scene where

For some weird ass reason, this achievement didn't pop for me, even though I've beaten the game. Guess I'll have to restart as a new character... I love this series (even have a vault symbol tattoo) but man is this game broken

Sorry to hear that, I've been fortunate enough where I haven't come across anything too bad yet but I know a lot of people are having issues with broken achievements.

Story Related, Can't Be Missed
This trophy will pop, once you return to Sanctuary III after killing the Vault Boss and have spoken to Lilith.
Boss details:
The Rampager:
The Rampager has the following attacks:
Fire Skulls
Radiation Skulls
Acid Skulls
Mutation (more on that in a bit)
Ground Pound
Flight
Fireball Bombing Run
Eridian Warrior Spawns
Rock Falls (these happen whenever the Rampager either jumps around or attacks)
This is the first of 4 Vault Monster fights in the game but it isn't that hard if you know what you're doing. For the first phase of the fight, you will need a decent fire weapon to whittle down the Rampager's health until he collapses and exposes the weak spot in his mouth but watch out for Radiation Skull and Ground Pound attacks since they will do a lot of damage to you if they hit. If you need more health or ammo, then kill either the Eridian Warriors or the little blue spirits that appear on the platforms on the left and right of the screen and pick up the items they drop.
Phase 2 will start after you have reduced the boss's health by a 1/3, at which point he will suck in all the Spirits and mutate into an even bigger and nastier version of himself that spits Acid Skulls at you but Radiation weapons will make short work of him so whittle his health down until he collapses again and re-exposes his weak spot. After he mutates for the third and final time, he will turn into a massive dragon that flies and jumps all over the map whilst raining fiery death on you so keep moving.
Phase 3 is the longest and toughest part of the fight, because the Rampager is now immune to all elements so only non-elemental weapons will work but his weak spot only opens up when he stands still and unleashes a devastating barrage of Fire Skulls at you so don't miss when aiming at it! The Rampager is at his most dangerous when he's flying around and dropping a barrage of Fire Skulls on you so keep moving in the opposite direction to which he's flying. After reducing his health to 10%, he will collapse and reveal the weak spot on chest so hit it with everything you've got. Once he's toast, enter the Vault and grab the Eridian Resonator then use it to reveal the lodestar in the back right corner of the room.
