This one is missable. I had to do a second playthrough just to get it.
After completing the elevator puzzle, the store owner will get out of the elevator and go to his store. When you are in the store and get the earmuffs, click on the yellow ball and you will be asked to complete a series of puzzles. Click on the bottom row of characters, on the one that matches the outcome of the question.
Making a mistake will close the game, and when restarting you will be at question 1 again. The questions also seem to be somewhat randomized so can be different the second time you pass them.
When you have completed the last question you will again have your doggo's yellow ball, and when you have saved him the achievement will unlock.
