Just a note that you cannot reload checkpoint for this, you have to restart the level. Also, I did not need to kill everyone, there was 1 enemy left at the end when it popped.

At the first section there are 3 enemies not 2 the one guy is to the left of the first guy that you kill.
You don't need to to kill the guy that walks to the two guys.

This isn’t too hard as long as you have some patience with enemy patrols.
With that being said it is still pretty hard to get spotted considering most enemies aren’t too close to eachother.
Crouching will reduce your detection meter.
Go behind a enemy and press RT to eliminate them.
You will reach a courtyard with 2 enemies to your left & 1 more moving from right to left. You can completely ignore these enemies and continue to the right to the upper deck where there will be 1 enemy patrolling.
You’ll reach a broken cabin with 2 enemies inside and 1 outside on the right.
Wait for the enemy on the right to have his back to you, eliminate them and the other patrolling guard.
You’ll them be able to double back to the front of the room and take out the last stationary target.
Assuming you were not detected you’ll unlock the achievement.

There's only one opportunity during a playthrough to pull this off, and you're armed with only a meat cleaver, so patience is most definitely a virtue here. During the 'Surrounded' mission, the opening features a fairly brief stealth sequence, during which time you'll only encounter about fifteen enemies, so if you're spotted by one, you'll need to reload your checkpoint. There are 18 enemies in total in that area, so you can ignore the 3-man-group chatting in the yard. All the others are easy to kill. Even if you screwed up and are discovered, you are still able to chop them down with 1-hit before they kill you.
