To unlock Fisherman you must first earn the “Baited” Achievement (Unlocking 2 Baits in 1 run). While playing as Fisherman I’d recommend buying “Bait” multiple times per post wave shop visit, as it ups your Damage %. Then win the run and the achievement is all yours.
Edit 1-As Pat macht MUH said, from the main menu you can go to options>Gameplay>Accessibility> Enemy Health/Speed/Damage down to 25% to make this a lot easier
