While this can theoretically happen naturally, it's unlikely to do so. It's easy enough to force though. I forced it by starting a run as Mage, and picked a ranged weapon (I used Taser). It's best to do this on Wave 1 before you have any HP Regen or Life Steal. Purposely run into enemies on Wave 1 until you have 1 HP left, and then it's really easy to avoid damage until time runs out, unlocking this achievement.
