Had to play the level a few times to get it to pop but the achievement is not broken

I was having the same trouble with achievements not popping. Dashboarded, closed the game completely and started over. Achievement unlocked after replaying the level. Same for other levels.

for the combo ones i just did hell endless mode, i got almost 10,000 in that game mode as there were so many enemies and power ups on screen that it's almost impossible not to get all the combo ones.
