I got this while hunting deer in mexico just used a xray power up and then got a headshot on a make deer.

can one play missions over once completeing the game? or do u have to earn all gold medals throughout the career without being able to return later? I know it has nothing to do with this chievo, but there is no guide for the medals, just curious....

The first opportunity I found to get this was on Mission 16. This is when you can't move, and you can only adjust your angle of firing. Deer will come from the left of the screen, down a slope to the right, and then behind some trees.
Keep replaying this mission until you get a 5 star 100% integrity kill. The achievement will pop within a couple of seconds if you do this successfully. Keep retrying this mission until you get it. Don't forget to get a gold medal before proceeding to the next level, also.
