I had 70 % Occult and didn't get all the occult items....

I don't understand how this achievement work. At the end of my playthrough, I had Farewell, Fight Destiny and The Call of Cthulhu.
I drink, I read all of the unholy books, I spoke R'lyehan every time I could and I did not save Cat.... And I had around 70% in Occult

All the Hidden Items are showing up based on a skill check in the game. Theoretically you can have it at 30% also. Just reload the Checkpoint as many times as it takes... Same with the Padlocks.

Also I did have 100% occult at the end once but did not get the Farewell ending. So it must be based on something else

I did everything you listed and got the option to refuse at the end. I had 100% Occult as well. It might have something to do with choosing all possible Occult answers over the course of the game as well. +1.

I did exactly what Jotuns360Tag described, with the exception of the jail cell. I struggled and refused the knowledge. However it still worked perfectly. Had all 4 options at the end.

I also followed the steps that Jotuns360Tag described, and it also worked for me. Well done.
I'd like to add something that I believe is not key to unlock all 4 endings, but may be interesting for those who also want to go for the R'lyehian answer achievement in that playthrough.
I did follow Jotun's descriptions to the letter, with the exception of using the gas diversion in the asylum. I once used the electric diversion but did not lose additional sanity so I don't know how that works.
Anyway, in chapter 9 when you are back at the mansion. The R'lyehian answer to give Algernon was locked for me at first. In fact, I had to investigate Sarah's ritual and then when I returned to hand over the amulet I still got the option to give that R'lyehian reply.

I followed Jotuns360Tag's guide, but was also trying to complete additional achievements. I did reach the unholy books (another guide had said this was fine) and Irene was not driven to suicide. I don't know if one of those screwed it up, but did not unlock. Oh well.

I was going for this achievement and chose to refuse the ritual. For some reason, the achievement did not unlock for me. After a hard reboot of my Xbox and a checkpoint reload everything worked out as it was supposed to.
Seems this achievement is potentially glitchy.

Just finished for the 4th time and still no "refuse the ritual". How can it be there are 2 guides that are polar opposites of one another, drink, dont drink, submit to leviathan, dont submit, save cat, no save mitchell. Literally every fucking option is different, now which is correct because ive done both to no avail

I tried to do it the way the achievement walkthrough described the first time and the second time I tried I used this guide. Both did not work for me. Instead, I used the guide that was published on xboxachievements.com. Basically, :
-Never drink
-Pushing Irene to the limit
-Accepting help from and saving Drake
-Reading all unholy books
-Choosing the first 4 possible answers in R'lyehian
-Struggling with the leviathan and refusing the truth
-Sparing Bradley
-Saving Cat
-Refusing medicine, flesh and gun
-Calling Sarah crazy
In addition I went for all the occult objects (which I ended up with about 65%). My character was psychotic in the end, btw.

Pretty sure only chapter 9-14 are the only chapters you need to do for the endings

I got it using Jotuns360Tag guide. I also collected the occult items but I failed a 1 or 2 R'lyehian checks as my sanity was too high during chapter 9 I think it was. I did have all 4 options at the end. Oh and Irene did not die in my run and I didn't investigate Sarah's room in chapter 9. I finished wuth only 60% occult I think.

I just got all 4 endings using the same steps as Jotuns360Tag (rewritten under Playthrough 2 of the walkthrough). I believe the key to getting all four endings lies in not reading the occult books. I did a second play through to where I followed the steps put forth by Jotuns360Tag, except I did read the books and found all but one occult item. My third playthrough I did not drink or read the books, and I missed two occult items. Still managed to get all four endings.

I think you just have to have 100% Occult when you get to the end, which is tricky cause you need Spot Hidden at 50% by the time you get to the painter's bedroom to get the hidden sketch book and then have your Investigate at 75% when you unlock the cultist chest that's after the secret entrance in the study. Put all your points in Occult at character creation and then get all the Occult items.

(I posted this over at the 'Nameless Cults' cheevo as well :D)

Alright, I played this on PS4 rather than the XBOX, but still thought I'd add my choices. Seeing as I got access to all four endings on my second playthrough, and my choices were different from the ones stated..

- First of all, I put all points into 'Spot Hidden' on character creation
- I brought 'Spot Hidden' up to 100% as quickly as I could with earned character points
- I then brought Investigation up to 100% as soon as I could
- I divided the points after that equally across the remaining three categories

- Not only did I drink every time I could, but I exhausted the option to do so every time (3x)
- I think I found all the occult items

- I told Bradley to stay by the car
- I accepted Drake's help and saved him at the Shambler fight

- I interacted with all occult books, but refused to read them
- I started responding in R'lyehian as soon as I was given the option to do so

- I submitted to the Leviathan in jail and accepted the knowledge
- I killed Bradley
- I saved Mitchell and went back to check on Cat after

- I refused the medicine, the flesh and chose not to kill Fuller

- I chose prudence when it came to telling Irene about her husband but drove her to suicide for the trophies regardless
- I lied to Bradley about his ex, and gave him the R'lyehian response about faith after that

I did make some dumb calls that cost me some sanity along the way - like using the electricity to create a distraction, as well as investigate Sarah's ritual. But yeah, I had access to all four endings at the end, after choosing the final R'lyehian response.

Personally, I avoided anything could lower sanity, even without drinking.

Spending my starting points on Investigation.

Not responding in R'lyehian, nor reading any of the unholy books. But collecting the other occult objects.

Chapter 2: Take the backdoor into Warehouse 36, not the underground passage, an event occurs here that lowers sanity.

Chapter 3: Allowing Bradley along to the mansion.

Chapter 5: Do not enter the asylum morgue, even if bringing the asylum patient sleeping pills. (This means you'll have to use the gas as a diversion to escape as an electrical breaker is in here.)

Chapter 6: Go easy on Irene Sanders. EDIT: 20/12/21 I've discovered that partaking in the reconstruction in the living room is safe.

Chapter 10: Do not partake in the reconstruction in Sarah's study, resist Leviathan and refuse the knowledge. Save Drake during the banishment.

Chapter 11: Spare Bradley.

Chapter 12: Do not partake in the reconstruction in the Harbormaster's office and bringing the first-aid to Mitchell.

Chapter 13: Resist all of Leviathan's commands.

I did three playthroughs and by the end of the third I was presented with only two endings, Counter-ritual and Refuse.

During the final encounter of the game, choose "It's over." Make sure you follow the steps provided in Step 2 of the Road Map to have this as an option during the ending.

Missable.

Depending on your choices through the game, you’ll have different choices of dialogues at the end of the game. To be able to refuse to take part in the ritual you’ll need to:

-Chapter 10 to 14: Never choose the R’leyhian dialogues (unless there’s no other choice)
-Chapter 10: Struggle and Refuse the knowledge when talking to Leviathan
-Chapter 10: Save Drake during the Shambler battle
-Chapter 12: Spare Bradley
-Chapter 12: Give the first aid kit to Cat
-Chapter 13: Refuse the gun during the visions with Fuller

Then in Chapter 14, say “Are you crazy” when talking to Sarah and choose “It’s over” to unlock this ending.

Not sure exactly. But for me, the opportunity to refuse the ritual appeared only after refusing the knowledge that Leviothan offers (when you are locked in a prison)

Resist Leviathan, don't study occult books and objects, don't use Rlyeh in dialogues and then you can get this ending.

In my experience, it probably makes it easier to get the ending by not having Algernon on the cliff at the end of the game.

Let the Shambler drag him into the picture, then at the end Pierce will be able to abandon the ritual by pointing the gun at Sarah (and not at Captain FitzRoy, who threatened Algernon).

Got the opportunity to choose all 4 endings in one playthrough.

*didn't drink alcohol
*read all books without exception
*spoke in R'lyeh language only to Sarah and Drake
*Saved Drake, spared Bradley
*refused knowledge of Leviathan
*didn’t take medicine, didn’t eat meat, didn’t take a gun
*gave the first aid kit to Mitchell

To get the “It's All Over” (“Farewell”) ending, you need to maintain enough common sense at the end of the game, that is, not let your sanity indicator drop to zero. This can be achieved by meeting at least two of the following conditions:
do not read any of the unholy books "Hammer of the Beasts" (chapters 6, 7, 9);
keep Bradley alive (chapter 12);
resist Leviathan in all cases (chapters 10 and 13).

Driven me crazy everywhere. Only in chapter 13 did he give up medicine, meat and a gun (he didn’t drink alcohol)

I'll tell you how I got the trophy.

didn't drink.
I read all the occult books.
My sanity was completely gone.
refused the knowledge of Leviathan, refused to take pills, refused meat and a pistol (if you click, the game will still force you to refuse, don’t be afraid)
didn't save Joseph from the tramp
Spared Bradley
Saved Mitchell
did not respond to anyone in Ryelh language (not counting 1 plot point with Leviathan).

In short, to get this ending you need not to lower your ability to think (reason), and also give up everything related to cultism and expansion of consciousness. Upgrading the cultism parameter is acceptable, it is important not to lower your sanity.

What to do to get this ending:

- don't drink whiskey
- accuse Irene Sanders of sending her husband to the clinic
- read no more than one wicked book
- do not answer in R'lyeh (except once, when there will be no other options)
- when you find yourself in the mansion after saving Sarah, do not investigate the place of her ritual, so as not to lower your sanity
- accept Drake's help and don't let the tramp drag him away
- refuse knowledge of Leviathan
- spare cop Bradley
- refuse to take medicine, refuse to eat meat, refuse to take a gun (in the end, all this will be done by force, yes)
- in the White Dot, when talking with Dr. Colden (this is a vision), use the answer option like “Investigation”

In the final dialogue before the denouement, the answer "It's over" should become available.

I got this ending like this:
up to 10 ch.
during the passage, Irene remained alive (nervous breakdown?), read only two books with unholy knowledge, occult 75%, accepted offered help, did not drink, chose all available lines in R'lyeh, "participated" in all mystical scenes affecting the mind
from 10 ch.
-refused the knowledge of Leviathan and resisted him in every possible way (fought at the first meeting, did not take medicine, did not eat meat, did not take a gun)
-kept Bradley alive
- in conversation with truth and reality, looked at reality (many write that you need to choose between them, but no one says how)
-in conversations he refused to use occult remarks
- avoided the occult: missed the investigation at Fitzroy, did not look at the carcass of Leviathan at the whaling station.

I was the last one to get this ending.
-Dreka let a tramp drag him away
-Bradley was spared
-Refused knowledge (before that I clicked “Fight”)
-Did not answer in R'lyeh (except for the first time in prison)
-Didn’t take medicine, didn’t eat meat and gave up the gun
-Saved a policeman on the street from three infected people
-Well, in the end “it’s all over”

Completed all 4 endings at once by loading a save from chapter 10
I still don’t understand whether the actions before chapter 10 have an impact on the endings
