How to unlock the Shared guilt achievement in Call of Cthulhu - Definitive Guide

If you're following the amazing walkthrough provided by Nighthawx271 and Yinga Garten, you should be able to get this achievement no problem. However, if you didn't follow a walkthrough or otherwise don't have this achievement for some other reason, then the following are the steps that you'll need to take. Although the other guides listed here I feel are sufficient to be able to obtain this achievement, they have more downvotes than upvotes with no reason as to why, so I'm hoping that by providing a more detailed guide I may alleviate some confusion.

As per the walkthrough, there are 14 chapters to this game.

We first meet Irene Sanders at her house in Chapter 6.
When you sit down to talk with her, you will have a total of three conversation wheels pop up with options of what to say to her. Concerning the first conversation wheel, it doesn't seem to matter what you say, as the options on the second conversation wheel that appears later on will be the same regardless. In my case, when the first conversation wheel popped up, I chose "He died screaming, right before my eyes."
*Per Acidik Drake in the comments, choosing an option other than the one listed here for the first conversation wheel will not result in her committing suicide. Best to play it safe and choose this option.

The second conversation wheel is what matters for this achievement. You want to choose to say "It wasn't me that sent him there.." in order to blame her for her husband being at the asylum in the first place.

For the third conversation wheel, I don't think it matters which option you choose. I chose to say "Did he speak to you of a 'Shambler'"?

When the conversation ends, stand up and turn to the right. Walk past the phonograph and on the chest of drawers in the hallway just beyond the table with the phonograph is a recording cylinder that Francis Sanders recorded prior to going insane. Pick it up and go back to the phonograph to play it, and Irene Sanders should come over and listen. Stick around until the end of the recording and you should hear her say "I should never have let them take you away. What have I done? Everything is my fault." Make sure you hear her say that, so you know for sure that she is blaming herself for her husband's death.
*Per Leo Ascendent in the comments, playing the recording doesn't seem to matter, but I would anyway as it gives some interesting dialogue and background as to what happened to her husband.

The next important part comes in Chapter 8, where you'll be playing as Dr. Marie Colden. Progress through the conversations with the Nurse and Dr. Fuller at the beginning until you're able to move. In the room you start out in as well as in the room to your left are patients. What we're interested in here is a bed in the middle of the second room that belongs to the body of Irene Sanders. If you're leaving the first room and walking towards the desk located in the back corner of the second room, her bed should be the second one on your right. Inspect her body and medical report to find out she committed suicide.
*Per ASUknown1 in the comments, he completely missed looking at the body, but did everything else and still got the achievement. Don't take that for granted though. Probably best to play it safe and examine her body regardless.

Note: examining all patients/reading their medical reports and interacting with them as well as the two men on a bench out in the hallway will net you the following achievement.

As of this point in the guide, you've done everything to earn the achievement, but it won't actually unlock until Chapter 10. During your first conversation in the station with Officer Henry West, he'll talk to you about why you were arrested. The achievement should pop up during this conversation, the moment he mentions finding Irene Sanders dead in her drawing room.

**Important**
On a second playthrough, in chapter 6 I did not blame her for her husbands death, but I did play the phonograph recording. This choice still resulted in her ending up in the hospital in chapter 8, but she was alive and to be transferred to a mental ward. In this case, you don't get blamed for her death in chapter 10, so you won't get the achievement.

Terribly sorry, that's very odd. I've tested the above solution twice on two separate accounts and received the achievement both times. Additional playthroughs to test the other speech options resulted in her being admitted to the asylum rather than in her death. What you say to Officer West plays no role in receiving the achievement, as he should mention her death prior to the conversation wheel appearing. Once he mentions her death, you should get the achievement. What did West end up saying to you?

Nope, this explanation omits the same information the walk through does.
What option leads to West mentioning Irene because he didn't for me despite following the walk through to the letter.

I didnt play the audio for her, but did blame her for sending her husband to the asylum. I also checked up on her while playing the Dr. Got it right away

I do think your first response to her matters. I followed the guide, but did the middle response where I lied about not seeing what happened. Then I blamed her with the "I'm not the one who sent him there" and played the recording. This didn't get me the achievement as she didn't commit suicide. However, once I chose the "He died screaming" option and kept the rest the same that got her to commit suicide in my game.

When you meet who choose all the options that point the finger at her like the option "i wasn't the one who put him in there.." go over to the table in the hall pick up the voice can and play it. Later when you are the Dr go up to her bed read her clipboard and click on her body. Later at the police station it should unlock for you!

You'll know you're on the right track when Irene Sanders says "What have I done? Everything is my fault." after listening to the cylinder recording. After that:

i didn't have any special stats i chose chat options that came with the normal character no special skill options. It unlocks in chapter 11 when you get put in jail

Hmm. I chose the "I wasn't the one who put him there" option and played the recording in her presence and she was in the hospital but alive. There must be something I'm missing or this needs to be done on a different path.

This achievement requires three actions be taken over multiple chapters in the game:

  1. Chapter 6: Make sure you imply that Irene is to blame for her husband's death when speaking to her (choose "he died screaming" and "it wasn't me who sent him there"), then play his recording on the gramophone.
  2. Chapter 8: Find her body and inspect her chart in the hospital.
  3. Chapter 10: The achievement will unlock when you are imprisoned and accused of causing her to commit suicide.

Missable!

This trophy requires you to perform actions in Chapter 6 and Chapter 8, and it will then pop in Chapter 10.

In Chapter 6, you’ll eventually encounter Irene and will be able to talk to her. Make sure to choose “he died screaming” and “it wasn’t me who sent him there” when talking to her.
After that, go behind her and pick up a cylinder on the desk and play it on the gramophone.

In Chapter 8, you’ll play as another character in a hospital, and in the middle of the area you will find Irene’s body. Interact with her body and with the chart at the bottom of the bed.

Finally, the trophy will unlock at the beginning of Chapter 10, during a conversation, as long as you followed these steps beforehand.

During a conversation in the mansion with Bradley and Dr. Colden, select “Tell the truth.” Then, during a conversation with Irene in Sanders’ apartment, we tell her about the death of her husband and reproach her, we also find Sanders nearby and let her listen to the recording. The trophy itself will only appear at the police station .

The trophy will drop at the beginning of Chapter 10, when the local police officer accuses you of the death of Irene Sanders.
