After destroying the three glyphs and returning to Algernon Drake the final time, the Shambler will be dragged into the painting via interacting with the painting. During this time, it will grab onto Drake's leg -- simply press
to save Drake before he too enters the painting.If you save him, Drake will be at the final summoning ritual in Chapter 14 with the R'lyen Key. This will allow you to select [Perform the Counter Ritual] as the last option of the game. Achievement unlocked.
(NOTE: Once you finish the game, you can reload your save to get any other ending available as part of the last option.)

During the final encounter of the game, choose "[Perform the counter-ritual]." Make sure you follow the steps provided in Step 1 of the Road Map to have this as an option during the ending.

Depending on your choices through the game, you’ll have different choices of dialogues at the end of the game. To be able to refuse to take part in the ritual you’ll need to:
-Chapter 9 to 14: Always choose the R’leyhian dialogues
-Chapter 10: Submit and Accept the knowledge when talking to Leviathan
-Chapter 10: Save Drake during the Shambler battle
-Chapter 12: Kill Bradley
-Chapter 12: Give the first aid kit to Mitchell
-Chapter 13: Accept the gun during the visions with Fuller
Then in Chapter 14, choose to “Perform the counter-ritual” to unlock this ending.
