I saw it unlock for me when I completed this chieve, but now it shows locked again and is not completed. I tried to completed it again and still no unlock. Anyone else having this issue with this chieve?

I couldn't get this to unlock at all (completed the campaign several times). So I deleted my game save data both locally and on the xbox network and restarted my console. I started a new game and when I got to this point just before the moon assault, once I added the customised loadout it actually popped. Thank god for that.

This trophy is earned by selecting a custom weapon from the armory before a single player mission. The bare minimum you can do to earn this trophy is to change one of the attachments on one of the recommended weapons in the armory screen. You don't need to set up a whole new loadout or take a different weapon.
