Operation Blood Storm is the seventh and final campaign mission, so this is effectively a 'complete the game' achievement. Finish the mission and the achievement will pop.

Didn't unlock for me on my regular playthrough and didn't unlock again when restarting at the last mission part. When exactly is this supposed to pop?

Story related, cannot be missed.
This trophy is earned for completing the seventh and final mission of the single player campaign, Operation Blood Storm.
There isn't any continuation after finishing this last main mission, so if you want to finish the remaining side missions if you didn't do them all previously you will need to complete them through mission select.
