I completed this mission on Veteran and then proceeded to finish the whole game on veteran, however, looking at my achievement list I realised that this achievement didn't unlock :/ should I replay the mission? Does anything prevent this achievement from unlocking?

I don't know. I haven't encountered any bugs. I'd just replay it. But if you got Ends Justify the Means then I wouldn't worry about doing it on veteran and just go on casual

This is a story-related achievement, you can't miss it. You will earn it at the end of the mission "Contigency". If you encounter some troubles, just follow this video guide (difficulty is set on veteran, but the given tips are useful).
