This can be earned on the mission The Only Easy Day... Was Yesterday.
You can get this in any of the breaches you have to do, but I think it's easier if you do it on the first and last breaches. The last breach of the mission is a bit easier as they are all in front of you after breaching.
If you think you've missed an enemy you can revert to last checkpoint.

first breach, left door. EASY

If you do decide to breach on the right door, don't be an idiot like me. There are three guys in close proximity to the table in front of you. The one on the left is crouched down at first and his backpack is not part of his hitbox causing me to mess up the achievement like 3 times.

Mission 9 The Only Easy Day... Was Yesterday
Difficulty: Recruit
There are multiple slow-mo breaches during this level. The easiest is the first one, as this is the game's introduction to the mechanic, and has more than four enemies. Simply focus on your accuracy to eliminate four enemies with four bullets. Reload the checkpoint as needed.

To get this Achievement you must shoot 4 enemies with 4 bullets during a slow motion breach.
A good level for this Achievement is The Only Easy Day Was Yesterday since you breach about 3 times during the level.
The video shows the second breach encounter & watch out for what is on the other side of your shot since you could accidentally do friendly fire.

The first opportunity to unlock this is in Mission 9, 'The Only Easy Day... Was Yesterday'. A slow-mo breach is when you breach into a room with the wall explosive.
Once you are out of the water, follow Soap up the stairs and at the top you need to take out an enemy. Once you do, on the left you will see two doors with the word 'Breach'. Move to the left door and hold
to place the charge. When it explodes, you will move into the next room in slow motion. In front of you there will be four enemies and two civilians (one at the left-most position and the other in between the first and second enemy [going left to right]). From the time you go through the door to when slow motion ends you have approximately 7 seconds. Carefully but quickly shoot each enemy only once (they die with one shot). If you killed each enemy with one shot each, the achievement will unlock. If not, quickly reload the checkpoint and try again as there is a checkpoint just before posting by the door to breach.
Note that if you complete the breach and the message 'Secure the hostages' appears on screen, you will receive another checkpoint. If the achievement didn't unlock here and you missed reloading, there are two more breaches in this mission where you can attempt this.
