This is completed during the first mission of the game; S.S.D.D.
As you complete basic training and enter The Pit, instead of listening the dialogue to run the gauntlet, look up in the observation nest, and shoot the General in the middle, killing him and failing the mission. Achievement Unlocked.

This can be earned on the mission S.S.D.D.
Once you reach the training course where Cpl Dunn is turn to your left and look upwards. You'll see General Shepard standing over looking the course. Simply shoot him with any weapon and you'll be given the friendly fire is not tolerated message. After this the achievement will unlock.
If you're still struggling, feel free to use this video made by BreakingBear101 as a reference.

Very easy achievement. During the tutorial simply walk through the entrance gate at the practice course, look up and shoot Shepherd
To get this Achievement you must shoot General Shepherd on the level S.S.D.D.
An easy way is in the Pit with Cpl Dunn since he tells you where Shepherd is.

In S.S.D.D just as you enter the pit he is standing in the middle of the three guys watching you, directly above the gate that opens to let you in, just shoot him and unlocks the trophy.

Here's my quick guide on how to get this done! Hope it helps! :)
When you're playing Mission 1, 'S.S.D.D.', enter The Pit training course and instead of going through the open gate to start the course, look up and you will see three officers observing you. The middle officer is General Shepherd, one of the main characters of the game. To unlock this, simply take aim and kill Shepherd. You will get the "Friendly fire will not be tolerated" message, but it'll be worth it.
