Load Stalingrad. After you complete the objective of "Make it home", you keep going forward and a German soldier yells, "Get back inside!" or something like that, follow his demand on where to go and very shortly you'll enter a crawlspace. Then, you'll be able to pick up a knife behind an enemy soldier which immediately saves as a checkpoint I believe. I killed him before attempting what I'm about to state. Killed him, then reloaded my last checkpoint (which is when I pick up the knife), then quickly crouched into that last crawlspace for a few steps, then immediately upon exiting the crawlspace went for the kill on said soldier. Reloaded and kept doing the same and my achievement tracker increased upon each kill.

I thought I had "completed" the "Made it home" objective. I may not have yet I just found out. My guide takes place basically after you jump into a building which then some wood falls on your head and your character passes out for a small time. You'll know what I'm talking about after playing. Apologies on my misinformation if there were any. I posted my guide on my first playthrough and saw that nobody had posted anything as a guide yet, so I figured I would.

In the mission “Stalingrad” after you
You’ll quickly come upon some tanks driving and get the objective “Avoid the tank column”. Before crossing the tanks path, you’ll see a military vehicle to your right. Simply crouch down under it and at the end of the crawlspace once your standing two soldiers should be to your left. Kill them both and then reload your last checkpoint. Rinse and repeat until it pops.
(Note: if you are not fast enough from when you load in up to the point were you get to the enemies it is possible they could have moved away. This is not hard to do in time or anything I just thought I’d address some possible confusion.)

This can be done during Mission 3: Stalingrad or Mission 6: Lady Nightingale (but mission 3 is best).
Each of the characters in the campaign will have special abilities. Polina can crouch and crawl through vents and other tights spots like this very fast.
During the 2 big encounters in mission 3, there will be a lot of crawlspaces and for this trophy you need to kill enemies shortly after getting out of these spots. Make sure to shoot at the first enemies you see when getting out of your crawlspace and then restart the checkpoint to farm this trophy and get it out of the way.

You will play as Polina throughout this entire mission, and at one point will be told about how you can crawl quickly with her through small crawlspaces. As the description says, you need to kill a cumulative total of 15 enemies "immediately" after emerging from a crawlspace. While it's hard to put a time limit on this, my experience was that you had around three seconds after exiting.
During this mission, your first opportunity will come after going through a vent that ends with a grating you need to knock off (and a few enemies walking around on the other side of it). Because this achievement is cumulative, you could actually utilize this checkpoint to grind out this achievement right here: knock off the grate, come out of the crawlspace, shoot 2-3 enemies, then Load Last Checkpoint, and repeat until the achievement unlocks.
After that, there is a large fight inside a big open area, with tons of enemies inside, and crawlspaces lining the outside of the entire area (not to mention tons inside the area itself. You can also use this opportunity to purposely pop out of crawlspaces right before shooting someone, and there are far more than 15 enemies in this section.

Do not confuse a low squat with a regular squat. When you crouch down to some low passage or hole in the wall, Polina automatically sits down even lower to crawl through. We stop next to such a place and begin to crouch, shooting enemies.
