I found 2 struggles in "Death factory" mission: first one into the left stable as in Powerpyx's video and a "bonus" struggle into the right stable (it's on the right as soon as you go in). So you'll be able to get the achievement even if you miss one struggle.

There are only 10 Struggles. I found a guide that works way better because all mementos and heroic actions are shown at once. For the Ambush struggle, I got confused at first but you have to provide overwatch for Pierson first.
