In the mission "Dark Water" you will reach a set of stairs shortly into the mission after you clear the bottom deck. As you reach the top of these stairs 2 porta-potty's can be found. Shooting into the left one will net you the achievement. If you take too long the enemy will leave the toilet, but you can easily restart to the previous checkpoint to try again.

This can be found at the very start of Mission 11: Dark Water. Once you clear the first deck, you'll make your way upstairs with Alejandro and Graves. As you reach the top of the stairs two hostiles appear above you on a balcony. Underneath that balcony you'll see two porta-potty's - the target we need to kill can be found in the left one. Just shoot him through the door before your team gets to him.
If your team gets there before you just "Restart Last Checkpoint" and try again. The last checkpoint is usually the top of those same stairs.

The COD MW2 mission 'Dark Water' has you, Alejandro and Braves boarding an oil righ in the Gulf of Mexico. Right from mission start, your going to clear out this red closet area with two enemies.
Head up the stairs, right after the turn on the stairs, a few enemies are going to pour out of a door. Alejandro and Braves should clear them out.
To the right of that door are two porta-potties, the enemy will be in the left porta-potty. Couple of shots through the door and the achievement should pop!

A very easy to miss achievement taking place on the Dark Water mission
When going upstairs, from the lower deck of the oil rig, the enemy is in the left side porta-potty

On the mission dark water ,near the start of the level ,when you go up the 1st set of stairs on the ship look to your right ,there are 2 porta potties shoot a couple of shots into the left one ,and like 30 seconds later the acheviment should pop👍🏼

During this mission, after you clear the first area and approach the clearing in front of the porta-pottiies, all you have to do is drop a mag into the porta-potty as you approach, there is no need to be sneaky or rush in ahead of the NPCs, your bullets can run faster and the enemy is in there from the moment you can see the potties.
The achievement never popped on my xbox (nor the floor is lava) but I heard and saw them pop on my phone while playing, they should pop immediately if you completed them accurately.
