This is a terrible achievement. I've just realized that when troops run up from behind the rock, they could be from any raft on the right that made it through, not just ones from the left. Still don't have it yet.

Lots of good suggestions here, I just want to add some details that helped me win the day:
Sit close to the TV. It might mean the difference between shooting at nondescript blobs of darkness and actually seeing rafts...especially for that dark spot behind the weeds/bushes on the left. Sitting closely, I never had to fire blindly in that direction just to be sure...I could see them when they were coming (but keep in mind that they can sneak up pretty quickly).
Aiming sensitivity: It's not a huge advantage to me, but it's something, so make sure you crank it up.
Keep in mind that it's probably harder to overshoot targets than to come up short because of how the projectile arcs...pretty much anytime I missed, I wasn't aiming high enough.
And the last, maybe most important detail: The pattern is the same every time, and when I was sitting very close to the screen I saw the one cursed raft that always got by me...towards the end, the last raft on the left behind the bushes is actually TWO rafts...and with all the smoke, I was missing the second one behind him every time.
Good luck. I had gotten the achievo for 2000 kills before this one popped, but I had it within 2 tries after I sat right in front of the TV.

I was having such a difficult time with this achievement, I dont know why but I was. I found out if I take as many out before they start paddling it becomes much easier. Be careful though as on the left hand side of the river there are weeds. These weeds disguise the rafts that you may have thought you had blown up.
If after you have taken out all of the boats and you see a select few enemies walking on the beach hit start and restart from the last checkpoint.
Patience is needed for this achievement, as I found out as it took well over 10 attempts.
Also if you are having trouble hitting them aim the cannon 2 cm above the targets. You will have a circle and FOUR lines around the circle. Make sure the line at the bottom is above your enemy and it should hit them easily.

Did you have problems with men spawning behind the rock? I'm 95% sure that it's a glitch because they come out from behind the rock and I can't see where they've come from. This achievment is one of the most frustrating for only 15g!

I didn't know if they were glitching or not, but im sure I missed a raft which was camouflaged by the weeds on the river. It is a pain in the butt achievement but its easily reloadable as the checkpoint doesn't engage until you have moved.

Some of these solutions are spot on. But below is a text guide that helped me a ton and was the reason I got the achievement. He explains through text of where to shoot first and were and how many rafts there are in that spot.

After you have killed the 3 together on the right, move left killing each one as you see it. Once you get so far left, you will see 2 rafts right next to each other heading here:

The rafts will appear pretty much exactly where the cursor is. If it doesn't then you were probably too slow. Destroy these two rafts as quickly as you can. I think these were rafts 13 and 14.
Once you have destroyed these, shoot the cannon just behind the bush in the picture to clear anything there. Then go over to the right quickly and finish off the last 2-3 rafts.

Just to complement what VenturaDK says, I find it quite efficient to aim a few centimeters above your target. In many situations, using the same space between the canon and the target works, no matter if it's to the right or left of the screen.
At the end of the first chapter you will need to use a Canon to fend off the troops as they make their way across the river. Aim higher than the rafts, roughly about an inch or so above to compensate the arch of the cannonball.
If your having a hard time with this increase your controllers sensitivity from the options menu.

This achievement is just a matter of patience.
Once you get to the canon, turn up the gamma as much as you can, if you use a TV-set, the rafts are difficult to see as long as they are on the other side of the river.
Keep strafing from left to right and back, aim a little to the right of where you want the cannonball to hit.
If you see any soldiers on your side of the river bank, load the checkpoint. Keep doing that until you know where they come from, there are about 6 routes they use to cross the river.

Every single time I was sure I got all the rafts, and every single time four soldiers came up from behind a boulder in the end - a quick glance on a few forums revealed that I wasn't the only one experiencing this problem.. I got it after quite a lot of tries, try shooting the weeds on the left hand side occasionally, even when you think nothing is there. Using this strategy, it only took me two more tries or something..

Just before your finishing killing the waves of rafts there will be a raft that appears behind the rock in the middle of the shore.
