To get this achievement, you have to play an online game with 3 opponents, but they don't all have to be human. You can set up a game with 3 players (or more), and when one person joins you can start the game. The remaining player(s) will be filled in with AI. Assuming you set the AI to a level you can beat, you can get this achievement fairly easily. With "easy" AI, the AI usually helps you build your cities. Once you complete a 9+ tile city, the achievement will unlock.

Must the city only be 'owned' by one player (i.e. only one player has a follower in the city and gets all the points)? Or can it be shared by multiple players (i.e. three players each have one follower in the city and so they all get credit for the points)?

Hard way: Just play the online mode in player match and hope you get players that doesn't know how to play it well.
Easy way: meet 3 friends and invite them to your game. have them help you by placing their tiles to benefit you. You can all trade for TOP OF THE HEAP and LORD OF CARCASSONE achievements

Again, very easy if done with a friend. Just have your friend place city tiles that will enlarge your city. If you both place a follower inside, you will both get the achievement.
