How to unlock the Ultimate Score achievement in Carcassonne - Definitive Guide
You can pick up an average of around 150 points if you follow a basic strategy against an Easy CPU opponent in basic matches.
When you place your first tile, place a pawn on the farmland portion of the tile. From there on, work at completing cities while building the roads around a perimeter as wide as you can. Try not to let the roads cut off the farmland from the cities, and build small cities. You will get three points per city that touches your farmland when the game is over and final scores are tallied.
On easy, the CPU doesn't appear to ever claim farmland, so you can also choose to place your farmland pawns in your final moves, but I find that doing it early encourages you to keep the land open, and prevents you from missing the opportunity by having your pawns tied up on incomplete cities or roads.
Monasteries can earn you up to 9 points each if you surround them with tiles, but if you don't, you'll still get one point per adjacent tile at the end of the round.

is there any way you can tell how many points you have so far? I can't find it.

I can't recall there being any way to check your "career total" points in-game. Best bet is to track it yourself offline, or just keep hammering away until you get it.

MOST IMPORTANT: I played the game a lot with my girlfriend, and it seems that locally, the score only counts for the total of the player that selects multiplayer in the menu, sets the match settings and starts the match. Other local players will NOT have their score added to their total. In addition, I suspect that your total score is saved separately on each console you play and NOT added up, so playing on multiple consoles may delay getting the achievement.
A little addition to the method mentioned above by Vermin360: If you have the King and Baron DLC, play king and baron matches against one easy CPU or a second controller. King and baron titles add a number of interesting tiles, as well as scoring points for completed cities and roads at the end of the game, greatly increasing scores for the round.
Also, avoid using the River/River II DLC. The river tiles will add little score to you in the end, and mostly the river will make cities and roads harder to complete. Farmland is also more divided so you will need to farm more for a comparable amount of score, and matches will take longer.
There is no way to keep track of your running total of points, other than manually noting it. Given an average of around 160-170 points per match, it will take some 30 matches for this achievement to unlock. This average is very easily achieved if you follow the above advice.

are you sure on different consoles it saves seperately cuz my game glitched and im hoping if i start again on another console i can still get it

Not much to say about this one. Every point you do counts towards it, doesn't matter if it is from a single player, a player or a ranked match.Once you know how to play you will probably average at about 130 points per game in single player with an easy AI player.
