How to unlock the 50 Shades of Red achievement in Carmageddon: Max Damage - Definitive Guide
For this achievement, you have to win complete all 63 events in the career. This means all events in 16 chapters, which are divided as follows:
• Ch. 1 – 2: 3 events
• Ch. 3 – 11: 4 events
• Ch. 12 – 15: 5 events
• Ch. 16: 1 event
However, the first event in each chapter is a “classic carma” event. Thesehave to be won may be completed three ways, “Race, Wreck or Wreak Havoc”:
1. Simply race through all checkpoints
2. Wreck all your opponents
3. Kill all the pedestrians on the map
While the first two challenges shouldn’t be a problem at all, the last one could be tricky: In each map you'll find about 200 – 800 pedestrians, which have to be killed separately. Because some of them are scattered and well concealed, this task could be very time consuming.
The best vehicle for this is the “Electric Blue”, which could be unlocked later in the career (Stella Stunna is its owner). Although the car itself is very weak, it has a built in “Electro-bastard Ray”, which will kill even large groups of pedestrians with ease.
The next advice is to find the “All peds on map pups”, which are very well hidden and hard to reach, but present on each of the maps. There is a YouTube-playlist (https://www.youtube.com/playlist?list=PLUFfJtBb86JCWrXcrT92V...), which shows how to get them. Credit for this goes to Rattamauled (http://www.carmageddon.com/users/rattamauled)!
So finally have fun grinding some peds, wrecks and checkpoints!

Thanks to Smegged for sharing his info. Originally, I was posting all events have to be won. Actually, you just have to complete each race to meet the requirements.
However, this achievement is known to be very, very buggy. It may or may not unlock for you when completing the last race in chapter 16. Don't blame me, blame the developers.
The only solution I'm aware of is to start a new career in the second or third of your three possible game slots. Don't delete your save game of the first run, just to be safe.
For me, this unlocked around Chapter 10 of my second play-through.
Another note:
You may repeat races to earn more credits to unlock the next chapter. These credits are cumulative, so replaying races adds those extra credits to your overall account!

I played the campaign two times

I did not need to complete the classic carma races 3 ways. Just completing them one way plus completing every other race in the campaign was enough to unlock the achievement.
Thanks for sharing your info, I'll update the solution. To be honest, since not unlocking for me at all I was not 100% sure what the requirements are.I did not need to complete the classic carma races 3 ways. Just completing them one way plus completing every other race in the campaign was enough to unlock the achievement.

Complete all the events at least one way ( the classic carma events don't have to be completed all three ways when it popped for me)
* This not so much a guide, so no need to give any review of it... but it might be worth going for normal, instead of having to do the races over. That I have to now
What I've experienced was, that it wasn't counting on my favourite difficulty.
Pulping puppies..
Not because of the difficulty itself, but the animation :)
Noticed it around chapter 3, and changed it to normal. And it started going up in the %
At chapter 11 and on 48% atm

What do you mean by "going for normal"? And what animation?

Simply complete all Events in Career. You don't have to complete all variations of the Classic Carma events for this to pop but you will have to later anyway to get all the cars in the 100% career trophy. For tips on killing all Peds in Classic Carma levels. See Here in my cars for details on Ped barrels.

It is necessary to complete all missions in all chapters, not just the last one.
