I had a hard time getting any perfect quick starts until I switched to Mater (Dutch: Takel), it was far easier to get a perfect start with him.
To get the maximum amount of time remember the Intro sequence is always: your car, 2 other cars, your car again. At the moment you car appears on the screen for the second time you should start smashing the RT button.

I'm not sure why anyone would vote this negatively.
As soon as I followed this advice I was getting them every time with ease, so I recommend using this technique if you don't own a turbo controller and have to do all 40 yourself.

Just to add to this - you can press start then 'restart' after each attempt and it will save your progress. You don't need to finish the race or exit fully.

I was glad that the game has some achievement tracking for EAT MY DUST. In the awards menu under "Crests" it has a ticker to give you a reference to how many more perfect quick starts you need. If you are using the turbo cheat, I highly recommend you turn it off. I never knew how far I got until I turned off the cheat. You don't need a turbo controller, but my arm would disagree with this statement. Don't be afraid to take a break.

Also did this without a turbo controller, it's not that tough. If you have more than one controller don't be afraid to try a different one. I swapped my favourite controller for another older one and the right trigger seemed more responsive on the 2nd controller and the quick starts felt easier. Worth a go if you're struggling and have a pile of controllers kicking around like me.

I have a turbo controller but it does not have turbo for the triggers :/

I found a very useful tip to get these without a turbo controller.
Select Mater and start pressing when the camera zooms into your car the 2nd time.
Next you should press the trigger fully then not fully release it. Release it a few millimetres then press again. Doing this requires less movement and a leads to a higher amount of presses. 90% of the time I managed a perfect start using this method.
Also press the trigger on the end or opposite the lettering and ensure you finger remains in contact with the trigger throughout.

To add too the above solutions. You can also get this much faster with a buddy and a second controller. Go to free play chose either race or battle race, both of yous start mashing when the game tells you too. Then as stated above restart the race. Its getting 2 a time making the process too 40 much faster
I've come up with a solution that makes it MUCH easier for people without turbo controllers. Like the other people above use Maher but instead of using your finger rest your controller in your lap and keep pressing RT with the edge of the palm of your hand (about where it meets your thumb) and you'll find that you'll be able to press it faster and if you keep managing to do it and you keep restarting then you'll have the achievement in no time!
However be careful if someone walks in on you using this method as it might look like you're doing something else ;)

This solution is for backward compatibility users and potentially Xbox Elite controller users.
Just remap your right trigger to the A button and vice versa in the Xbox settings for your controller. Then, when it comes time to mash the button you can do it with A. It is so easy with a button versus a trigger. I got this each time I tried.
I have an Elite controller, so I put this on profile 2, so I could just quickly switch back and forth. Either way, this is still a grind to do it 40 times but this made it actually possible each time I tried.

Found a relatively easy Non-Turbo controller method that won't kill your arm muscles (was kind of a combination of all the help from previous comments btw).
First, find your average rubber band. Wrap it twice around the controller, but only use one band on the trigger (so it doesn't press the trigger completely down, only half or most the way down).
After doing this, it seems that for every push your finger does the game is counting it as two pushes (thanks to good ol Mr. Rubber band). I got this achievement pretty quick without killing my arm, hope this helps!

Best way I found to do this without a turbo controller is to hold the controller upside down and have one hand covering the front of the controller and the other hand on the back (without pressing any buttons) and your thumbs at the bottom of the controller (which is facing upward) Then simply rest the trigger up on your leg and start mashing. I was severely tensing up to make my nerves shake my hands to bash that trigger against my leg. :)

The other solutions are all great but getting to 40 with them was just not working for me but I've found a very simple method using an Elite controller.
customize your paddles on either the left or right (depending on what hand you prefer) and make BOTH paddles "RT"
I used Mater and rapidly tapped both paddles back and forth using the first 2 fingers on my right hand and was filling the meter completely very quickly, sometimes even before the countdown hit 1.
the paddles need a significantly softer finger press compared to any other buttons on the controller and having 2 RT's makes this incredibly easy.

Before a race starts, at the bottom of your screen, it will say “REV”. Press as fast as you can and if you fill up all four turbo bars, that is a perfect start. You can get a perfect start, then just select restart and do it again and again. This is cumulative and counts across all play modes. Your progress on this achievement can be tracked by selecting "Awards" from the main menu.
To do this manually, you want to tip your controller vertically and use your index / pointer finger to rapidly press down on the trigger. It's not too difficult once you get the hang of it.
NOTE: A Turbo Controller will make quick work of this achievement, so use it if you got it!
