How to unlock the And then they all die achievement in Castle of no Escape - Definitive Guide
UPDATE: As of today's update, the game has been patched, and the achievements are now available!
If yours don't pop, I did had to uninstall and reinstall my game before they started working, but with the game only being a couple of hundred MB it's not too bad if that's the case!
Simply kill every enemy on a given floor.
When on the Character Select Screen, hit . This gives you the best hero, starting with 20/20/20 stats!
It will also net you this achievement.
When fighting, hit , this opens your spell menu. Just use Fireball . It one hits most enemies, meaning you don't take damage.
When you get to a vendor, buy whatever stat you have lowest. Try to keep your health over 20 at all times.
In looking for all the enemies on a floor, you will be very close to getting this achievement, so you might as well clear the last few squares for the extra 200G!
EDIT: As pointed out by Patrizia84 in the comments to my other solution, if you are blind, the "Been there, done that" achievement doesn't unlock. However, if you're blind, you can press if you have a flare (torch icon on the right-hand side of the screen above your stats) you can get this achievement as well:

To unlock this, you need to kill every enemy on the current floor. This may or may not unlock before you reveal all the spaces on the floor (for Been there, done that (200G)). Refer to What a hole (200G) for information on sinkholes and warps.
An easy way to do this is to select the secret character by pressing on the character select screen (to unlock But there was a fourth one (200G)). With this character, all three stats are maxed out at 20, making you more powerful than the other starting characters. To reveal the spaces, you can work in a pattern or randomly as they accomplish the same thing. When you reveal an enemy, you have two options; to ‘Fight’ or to ‘Cast’. Always select and cast a Fireball by pressing . This usually kills enemies with one hit, sometimes two or three. Each time you attack, you lose 1 Intelligence.
As you move along, you should reveal a vendor or two. Use this opportunity to purchase any stats (if you have the gold) that may have diminished while fighting enemies (you can always go back to them when you have obtained more gold; each stat costs 10 gold and you recover 3 of the stat you choose). You should also be revealing books and ancient chests, but don’t open them if you do not have any flares (refer to Come again? (200G)).
Once every enemy on the floor is eliminated, the achievement will unlock.
Note: The ‘boss’ of the game, Nikolai, may appear on any floor. If you happen to reveal and kill him, the game ends, so if you do come across him, do not fight him right away. Instead, avoid him and move around the floor, killing every other enemy and leaving Nikolai for last. Or, you can attempt this achievement on another floor.
