The achievement pops when you open a chest and sustain damage. It is best to do this before picking up protective jewels and talismans that may protect you from damage.
Note: This was the unlock condition when the game was first released. Later, the achievement was changed for standing still for ten seconds without any button presses. Now, the unlock conditions have reverted to the original. Since you must play online and the game will not launch unless the current version is installed, the solution described above is the only way the achievement can now be unlocked.
