The only missable soul in the game is Legion. While this boss has a 100% drop rate, it will only drop if the entirety of the outer shell is destroyed. It's possible to hit the core while the shell is still up, so while you can use things like Beam Skeleton or Lightning Doll to get multihits on the shell, be aware that you may kill the core ahead of time if your aim is a little too good.

While other bosses do not necessarily drop their souls, it's because they can be found as generic enemies later in the game.

SETUP

EQUIPMENT
Weapon: Claimh Solais
To get this, you will need to enter the Forbidden Area. In the Underground Reservoir, there is a tall room with a waterfall. Equip Undine to stand on water, then either Curly, Devil, or Manticore for a charge ability. Use it as far back as possible; the ability will cancel when you hit the waterfall, but you should have enough momentum to carry you to the other side. Continue through this area until you get to the sunken ship. Equip Skula, go to the bottom left, and break open the wall for a secret room containing a holy greatsword.

Armor: Dracula's Tunic
After defeating Graham in the Top Floor, this is found behind the boss room along with the Black Panther soul.

Accessory: Soul Eater Ring
This costs 300,000 gold at Hammer's Shop. Sell anything you have no intention of using and this should bring you close to the limit. If this still isn't enough, get the Mimic Soul from Dance Hall, Forbidden Area, or Chaotic Realm; this soul gives you 5 gold for every point of damage taken. Go to the bottom checkpoint in Clock Tower and sit on the spikes to rack up the money.

Additional Equipment:
Get a Mach Punch as a common drop from Werejaguar in the Underground Reservoir and Underground Cemetery, as this will be helpful later.

SOULS
Red: Use whatever you want.

Blue: Black Panther - Dash.
After defeating Graham in the Top Floor, this is found behind the boss room along with the Black Panther soul.

Yellow: Gremlin - Luck +8
Gremlins are the green gargoyles found in the Clock Tower.

Additional Souls:
Succubus is useful for staying healed, which you should already have. Alura Une is the best heal, and she is found in Forbidden Area.
Chronomage is found in the Inner Quarters to the left of the Headhunter boss fight. Time Stop Immunity is gained through the Galamoth Soul found after the Legion boss fight. This is required for a later soul.
Killer Mantle is found exclusively in the Arena, with an easy grinding spot near the save point closest to the boss fight. This is required for later Golem kills.

When looking for an enemy not on the list below, use the wiki if you're having trouble finding them. Typically, enemy IDs in the in-game bestiary are found by progressing through areas in story order.

Any Idea why I´ve got 100% and nothing popped for me? I tried to restart game and load up the save nothing

In response to New xDante1986, I had the same issue. You do need to get every ability in addition to all the souls. I was missing the Hippogryph, and despite being at 100% of souls collected, once I got it the achievement unlocked

For those that are looking to get this on New Game Plus because they missed the Legion soul.
The game takes away a number of souls from you that relate to abilities including:
Kicker Skeleton
Hippogryph
Malphas
Grave Keeper
Skeleton Blaze
Galamoth
Black Panther

My achievement popped after collecting Black Panther soul upon defeating Graham Jones (2nd form).

MISSABLE!

This can be missed due to the boss Legion. (Big floating corpse ball) If you destroy the core before knocking off the four shell pieces, he won't drop his soul despite it being a 100% drop rate.

For the other bosses, the black orb they drop is their soul or do you have to reset/rewind until you get their soul like with a nomal enemy?

No, the black orb just restores your health and magic. The souls drop directly from the bosses. The first three bosses(Creaking Skull, Manticore, and Big Golem)all show up as normal enemies later in the game, so those are not missable if you don't get them right away. Legion however, is a one time fight and will only drop its soul after fully knocking off its shell.
