To skip to the final boss, use password Eyeball, Heart, Candle, Heart.

I've beat this game several times and no achievement unlocks. What could be wrong?

Unlocked for me immediately after defeating him, before the usual epilogue with characters standing on the cliffside watching the castle crumble. Don't know why it wouldn't be unlocking for you.
Awful boss, even with save states, though this was probably because using the password to skip straight to the end meant I had no subweapons.
